POSITION SUMMARY:

The LPN Care Manager is an essential member of the Johns Hopkins All Childrens Care Network (JHACCN). They function as a resource for care coordination and care management partnering with other members of the team to address resource and health care needs for patients and promote quality care using a collaborative process that coordinates monitors educates and evaluates services needed. Their work includes analyzing andsharing performance reports with practices assisting with improvement actions working with patients/families providers and payers to close care gaps providing documentation in the population health database and other work to meet the goals of our population health initiatives.

Work Location: St. Petersburg FL (modified hybrid schedule after successfully completing orientation)
Work Hours: 8a-4:30p

QUALIFICATIONS:

Education/Experience:

  • Current and active Licensed Practical Nurse (LPN) in the State of Florida
  • Graduation from high school or the equivalent required. Associate Degree preferred.
  • Minimum of 3 years experience in a health care environment. Managed care experience preferred.
  • Must pass a Qualified Bilingual Staff certification of the JH Medicial Termnology modules and JH Professional Medicial Interpreter Training prior to offer.

Salary Range: Minimum 23.11/hour - Maximum 35.82/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
