Absolute Elder Care is seeking a Care Navigator (Remote LPN) to join our dedicated team in Ocala. This part-time remote position offers a unique opportunity to provide compassionate care and support to our elderly clients in a hospital and healthcare environment.
As a Care Navigator you will play a vital role in coordinating care for our clients ensuring they receive the necessary medical attention and resources. Youll work closely with healthcare professionals to develop care plans that enhance the quality of life for our clients. We offer a competitive hourly salary range of $35 - $65 along with flexible working hours to support a healthy work-life balance.
Responsibilities
- Assist in developing and implementing care plans for clients
- Communicate effectively with clients and their families
- Coordinate with healthcare providers to ensure comprehensive care
- Monitor client progress and report changes in condition
- Provide education and resources to clients and families
- Document all interactions and care provided
- Assist with medication management and administration
- Participate in team meetings and contribute to care strategies
Requirements
- Current LPN license in good standing
- Strong communication and interpersonal skills
