Job Title: Triage Clinician (Nurse or Paramedic) Walk-In Centre
Location: Walk-In Centre
Shift Pattern: 12-hour shifts (Days Nights Weekends and Bank Holidays)
Contract: Flexible / Bank / Permanent (Shifts available all year round)
Reports to: Clinical Lead / Service Manager
Job Purpose
The Triage Clinician will be responsible for the safe effective and timely clinical triage and assessment of patients presenting to the Walk-In Centre with minor illness and injury. The post holder will prioritise patients according to clinical urgency provide appropriate advice and direct patients to the most appropriate care pathway.
The clinician will utilise clinical decision-making skills and electronic patient record systems including SystmOne and Adastra ensuring accurate documentation and continuity of care.
Key Responsibilities
Clinical Triage and Assessment
Undertake comprehensive clinical triage and initial assessment of patients presenting with undifferentiated and undiagnosed conditions.
Prioritise patients based on clinical urgency using recognised triage systems and clinical judgement.
Identify patients requiring urgent or emergency intervention and escalate appropriately.
Provide safe clinical advice signposting or referral to appropriate healthcare services.
Initiate appropriate care pathways within scope of practice.
Clinical Care and Decision Making
Assess plan implement and evaluate patient care in accordance with clinical guidelines and protocols.
Recognise red flags and deteriorating patients and take appropriate action.
Provide advice on self-care health promotion and prevention.
Refer patients appropriately to GP Emergency Department community services or other healthcare providers.
Work autonomously within professional scope of practice.
Clinical Systems and Documentation
Accurately record patient assessments clinical findings and care plans using SystmOne and Adastra.
Ensure documentation is clear accurate and compliant with legal professional and organisational standards.
Maintain confidentiality in accordance with GDPR and information governance policies.
Communication and Team Working
Communicate effectively with patients relatives and carers.
Work collaboratively with multidisciplinary teams including nurses paramedics GPs ANPs and administrative staff.
Provide clear clinical handovers when required.
Escalate clinical concerns appropriately to senior clinicians or management.
Patient Safety and Quality
Deliver safe high-quality patient-centred care.
Adhere to local and national clinical policies protocols and guidelines.
Participate in clinical audit governance and quality improvement activities.
Maintain awareness of safeguarding policies and escalate concerns appropriately.
Professional Responsibilities
Maintain professional registration with the NMC or HCPC.
Practice within scope of competence and professional code of conduct.
Maintain up-to-date clinical knowledge and competencies.
Participate in mandatory training and continuing professional development (CPD).
