Reports to a Practice Manager/Director for administrative supervision and the Practice Site Physician or designee for clinical supervision.Is responsible forthe coordination and delivery of primary care as specified by theappropriate certifyingagency to BMG patients at designated work sites. Provides clinical and administrativeassistanceto BMG programs etc. Works collaboratively in the education of nursing students medical residents and other pre-professionals.

Coordinates and delivers primary and nursing care in collaboration with the Practice Site Physician by:

• Working within the approved scope of practice asdesignatedfor this type of professional.

• Obtaining complete medical history and physical data on patients.

• Interpreting and integrating data to determineappropriate diagnosticand therapeutic procedures as needed.

• Performing a complete physical exam and recording findings. Collaborating with physicians in managing acute and long-term medical needs of patients.

• Orderingappropriate laboratoryand diagnostic procedures.

• Synthesizing data todeterminepreliminary diagnosis and therapeutic plan utilizing principles of prevention.

• Administering medications and injections according to physician direction. Suturing minor lacerations.

• Interviewing and advising patientsregardinghealth and illness prevention. Recommending community resources to meet patient and family needs.

• Instructingpatientand familyregardingmedications and treatment instructions.

• Maintaining and reviewing patient records charts and other pertinent information. Posting tests and examination results.

• Triaging patient telephone calls and providing consultation.

• Coordinating or whenappropriatemanaging medical and surgical emergencies.

• Providing monitoring and continuity of care between physician visits.

Provides clinical and administrativeassistanceto BMG programs e.g. education follow-up and the like by:

• Participating in health education.

• Maintaining related records and statistical data and preparing statistical reports as needed.

• Participating in interdisciplinary care conferences andassistingwith referrals and dissemination of information to appropriate vendors.

• Assistingin the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.

Performs other functions tomaintainpersonal competence and contribute to the overall effectiveness of the department by:

• Assistingin development and complianceofclinical protocolspoliciesand procedures.

• Participating in quality review and improvement programs; monitoring quality and outcomes of carerendered.

• Communicating with physician(s) to ensure the quality and timeliness for servicesrenderedto patients.

• Performing otherjob relatedassignments and specialprojectas directed.
