Job Scope: The position primarily entails providing prevention resources STI andHIV/AIDS testing services for the risk population defined by the Center for DiseaseControl: Black and Latinx Men who have Sex with Men (MSM) high-riskheterosexuals and people who use drugs (PUD) in the designated service area. Provide and execute a plan to reduce health disparities and increase HIV preventionstrategies in targeted communities throughout Cook (non-Chicago) DuPage andKane Counties and complete required components of the HIV prevention YouthExperiencing Homelessness and Youth Homelessness Demonstration Program asassigned. This position is grant-funded and is subject to the availability of funds.
Work Location: 650 E. Algonquin Road Suite 104 Schaumburg IL
Service Implementation Area: Cook (non-Chicago) DuPage and Kane Counties
Primary Responsibilities
  • The position is not a work-from-home position. A DCFS background check must be
  • cleared to confirm a job offer for this position. Reliable insured transportation is
  • necessary for community outreach presentations meetings and community
  • engagement. Ability to work in a fast-paced environment where objectives may pivot
  • due to unforeseen circumstances.
  • Work collaboratively with various community sectors including local law
  • enforcement first responders social service agencies persons in
  • recovery persons using substances persons living with HIV/AIDS
  • persons experiencing homelessness and other community members to
  • achieve the projects objectives.
  • Participate in planned outreach and education in the community.
  • Work with staff to schedule program events organize logistics and handle
  • supplies and information.
  • Work collaboratively with the media department to produce prevention
  • education and resource materials within the scope of the project.
  • Work at identifying the needs of youth experiencing homelessness and provide street outreach (i.e. resources HIV testing community hotlines etc.) to meet those needs.
  • Work collaboratively with the supervisor to provide HIV and STI testing for
  • the target population.
  • Participate in appropriate professional development opportunities.
  • Attend mandated training and workshops.
  • Meet with a supervisor as scheduled.
  • Participate in required and supplemental professional development
  • opportunities.
  • Follow best practices as defined by the projects funder.
  • In compliance with grant rules proactively plan and collaborate in all community efforts with other division projects with shared objectives for
  • greater reach.

  • O. Work collaboratively with other KYC divisions.
  • P. Other job-related duties as assigned.
Administrative Responsibilities
  • Accurately collect quantitative and qualitative data for reports and other
  • documents required by KYC or the funder.
  • Prepare all required and supporting documentation requested by funders
  • and KYC.
  • Receive administrative input from the supervisor and provide feedback
  • regarding administrative issues.
  • Provide ongoing supervision and performance evaluation for supervisees
  • building on strengths and identifying ongoing growth/training needs.
  • Effectively implement and monitor program performance including
  • individual and unit productivity expectations.
Work Environment
  • While performing the duties of this job the employee regularly works in a
  • shared office setting and routinely uses standard office equipment.
  • Travel between offices and meeting outside of Illinois and to other locations
  • in the community will happen on a regular basis.
  • At times it will be necessary to attend meetings and events in the evening
  • and on weekends.
Travel Requirements
  • Have a valid drivers license own vehicle and proof of insurance.
  • The employee must travel by personal automobile to and from Cook (non1Chicago) County Kane County DuPage County and office locations
  • several times monthly as well as occasional meetings/training in and around
  • KYCs catchment area.
  • The employee is required to travel to statewide meetings and training in
  • Chicago Springfield Bloomington or Joliet (sometimes requiring overnight
  • travel).
  • Some out-of-state travel to national training may be required.
Minimum Position Requirements
  • Bachelors Degree in social services education or a related field or relevant
  • work/life experience is desired.
  • Experience in HIV Prevention and Youth Experiencing Homelessness fields
  • preferred.
  • Experience using Microsoft Suites and Google Drive.
Schedule
  • A. Non-Exempt Full-Time hourly 37.5 hours per week
  • B. Some evening and weekend hours are required based on the annual work plan and division program events; staff will flex their hours to not exceed 37.5 hours according to the agencys policy.
