Job Summary
The Service Coordinator role plays a vital role in helping individuals remain independent and engaged by connecting them to supportive services conducting non-clinical assessments and advocating for their needs. This position works collaboratively with program participants and community partners to promote stability wellness and quality of life.
Primary Responsibilities
- Complete and document non-clinical wellness and social assessments with individuals.
- Connect individuals to community resources such as nutrition programs (e.g. SNAP) transportation home-delivered meals and other supportive services.
- Maintain an up-to-date local resource directory tailored to individual needs.
- Collaborate with service providers health providers community organizations and other partners as needed to support individual well-being.
- Advocate for clients rights and ensure they receive culturally competent person-centered care.
- Provide education on emergency preparedness and assist with planning.
- Ensure timely follow-up on referrals and maintain accurate case records in compliance with program requirements.
QualificationsQualifications
Minimum Requirements:
- High school diploma or equivalent.
- 1 year of experience in social services case management support or healthcare coordination.
- Valid drivers license and reliable transportation with proof of insurance.
Preferred Requirements:
- Bachelors degree.
- Certified Senior Advisor (CSA) or Professional Service Coordinator (PSC) certification.
Skills
- Experience working with older adults or vulnerable populations.
- Strong communication interpersonal and organizational skills.
- Knowledge of community resources and social service systems.
- Ability to work independently while building collaborative relationships with individuals and stakeholders.
- Proficiency in Microsoft Office and case documentation systems.
Working Conditions
- Some roles are based in HUD-assisted senior housing sites and requires regular on-site presence to engage directly with residents. This role travels to COAAA for agency meetings and trainings.
- Some roles are based in office with some work out in the community.
