Collaboratingwith othermarketpeopleleaders the Sr. Manager Clinical Care willestablishmarket priorities and ensure an interdisciplinary approach for all role is responsiblefor overseeing the performance of individual clinicians bymonitoringthe provision of high-quality care and ensuring outstanding patient function will oversee Registered Nurse Care Managers (RNCM) and Licensed Clinical Social Workers (LCSW).This role will report to the Director Regional Clinical Care.
The Day to Day
- Ensuresclinical personnel within the regionfollowall policies and procedures.
- Closelycollaborateswith Market Operations Directors Implementation Teams Clinical Education Quality and Regulatory departments to support required training consistent skills development and process improvement initiatives for clinicians acrossthe region.
- Ensuresemployee competency through the development retention and performance management of the clinical the interviewing hiring and evaluation process of clinical personnel within the region.
- Manages the clinical team with direct management of RNCMs and general oversight of the performance of the greater market team.
- Accountable for managing and supporting clinical team daily productivity and removal of associated barriers to ensure achieving expectations.
- Fosterspositive culture and collaboration in the market to drive excellent patient experience through an interdisciplinary team approach.
- Collaborateswith Nurse Practitioner and CareCoordinator peopleleaders in the market to set priorities.
- Partners with Regional Medical Director to execute on initiatives to effectivelyimpacttotal cost of care andtoensure clinicalinterventions and campaigns are successful( CHFdiabetes) at managing the patient populations co-morbidities.
- Monitorsclinical quality metrics and patient care delivery reports toidentifyoperational issues or trends or progress in meeting performance measures and creates and deploys action plans to drive success.
- Implementsstrategies related to total cost of care medical economic and quality initiatives.
Minimum Qualifications
- Associatesdegree inNursing Healthcare Business or related field.
- Active licensure as aRNPhysicanAssistant Nurse Practitioner orotherclinically relatedlicensure.
- 7 years combinedofrelated education experience or certification.
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.
- Ability to travel and be on-site to meet business needs
Preferred Qualifications
- 1yearsexperiencemanaging direct reports.
- Experience managing remote teamseffectively.
- Understanding ofand experience inValue Based Care.
- Experience in chronic kidney and/or end stage renal disease.
- Experience hiring and interviewing.
