Kinwellis hiring a Clinical Care Registered Nurse to join our clinic inFederal Way Washington.

KinwellsClinical Care RN will provide comprehensive nursing care to a diverse group of patients and families across a lifespan in a community-based setting using the knowledge skills and abilities of the nursing profession. This role will have a deepened relationship with our patient population and in coordination with the Clinical Care team you will be responsible for monitoring patient health providing treatments administering medications and educating patients on health maintenance and disease prevention with the goal of improving health outcomes and facilitating access to services. This position will work closely with medical staff to ensure quality care is provided toKinwellpatients and will support their Clinician in managing a panel of patients.

Whatyoulldo:

  • The Registered Nurse is an integral member of the Health Care team overseeing and participating in patient assessment and the formulation and implementation of the care plan.

  • Collaborate communicate and coordinate with the clinicians and other health care team members in planning and implementing patient care.

  • In collaboration with clinic leadershipthe Clinical Care RNwill triage assess plan schedule coordinate evaluate and delegate the delivery of patient-centered nursing care in the primary care setting.

  • Identifyatriskpatients develops a plan of care and coordinates care with the patients involvement.

  • Standardize tasks and workflows that support all staff and providers working to the top of their licensure. This includes patient schedule scrubbing in-basketmanagementand medication refills.

  • Use technology such as televisits and remote monitoring capabilities to engage patients andmonitorchronic health conditions.

  • Administration of vaccinations and medications asappropriate. Performs testing as directed by a provider (e.g.EKG).

  • Supportfollowup patient care gaps (e.g.colonoscopies mammography A1c etc.).

  • Provide Transitional Care Management (TCM) following an inpatient stay for all identified patients.

  • Work withpatientto develop a plan to improve general health status.

  • Implement physical and psychological preventive measures.

  • Promote patient self-care responsibility for meeting their own health needs.

  • Assure quality of care through development implementation and evaluation of professional nursing standards and participation in multidisciplinary quality assurance activities.

  • Maintain conciseaccurateandtimelydocumentation that supports effective efficient management of the memberand thatmeets accreditation contractual and legal requirements.

  • Other duties may be assigned as necessary in the performance of this position.

Whatyoullbring:

  • ActiveWashingtonState RN licensurethrough the Department of Health (DOH)

  • Three (3) years experience as a RN in physicians office family practice or similar medical setting.

  • BLS current AHA HealthcareProvidertraining(BCLS).

  • Hasknowledge of Medicare documentation billing and reporting requirements associated with Annual Wellness Visits (AWV) Chronic Care Management (CCM)and Transitional Care Management (TCM) services to include patient calls and calculation of billable time.

  • Knowledge of care continuum (inpatient ambulatory post-acute etc.).(Preferred)

  • Experience with Epic Electronic Medical Record.(Preferred)

  • Bilingual.(Preferred)

Working Environment

This positionoperatesin a clinic environment providing both in person and telehealth care. Collaboration with various healthcare providers is routine and participation in wellness programs may also be required.

Physical Requirements

Ability to work in a clinical environment interact with patients for prolonged periods and perform duties requiringa high levelof concentration and emotional resilience.

The pay for this role will vary based on a range of factors including but not limited to a candidates geographic location market conditions and specific skills and experience.

National Plus Salary Range:

$92400.00 - $157100.00

*National Plus salary range is used in higher cost of labor markets including Western Washington and Alaska.
