About the Role
We are seeking an enthusiastic and experienced Registered Nurse leader to lead the establishment of a new private Hospital in the Home (HITH) service in Sydney.
This is a hands-on clinical leadership role suited to a nurse with strong operational governance and change management capability and a passion for developing services that deliver safe high-quality care beyond the hospital walls. While experience in Hospital in the Home or virtual care is highly regarded we welcome applications from nursing leaders with acute or sub-acute management experience across hospital community home-based or virtual care settings particularly those with demonstrated experience in service development and implementation.
Key Responsibilities
Program Establishment & Service Design
Lead the establishment and operationalisation of a centralised HITH program for St Vincents private patients.
Develop and implement policies procedures and operational systems for referral management patient onboarding scheduling subcontractor coordination and clinical care delivery.
Design and embed fit-for-purpose evidence-based models of care using human-centred design principles and strong clinician engagement.
Establish clinical escalation pathways including on-call and after-hours response models.
Build the operational infrastructure required to deliver the service including workforce models rostering training frameworks procurement consumables fleet and ICT systems.
Establish staff training clinical competency supervision and safety frameworks including on-road staff safety monitoring.
Actively promote Hospital in the Home services and grow the referral base across the St Vincents private hospital network.
Collaborate with national Virtual & Home program leaders to align systems processes and standards where appropriate.
Provide strong change leadership throughout service establishment fostering trust engagement and clear communication.
Ongoing Operational & Clinical Leadership
Provide day-to-day operational leadership of the HITH and Virtual & Home services including clinical workforce and financial oversight.
Lead referral intake and care coordination processes to ensure safe timely and efficient service delivery.
Deliver high-quality nursing care within scope of practice including assessment care planning treatment review and patient/carer education as required.
Build and maintain strong relationships with internal and external stakeholders to support service integration and performance.
Drive continuous improvement service development and innovation including the integration of virtual care models.
Lead recruitment workforce planning performance management talent development and succession planning.
Provide visible safety leadership ensuring WH&S risks are identified managed and escalated appropriately.
Lead quality improvement clinical governance and compliance activities in line with legislation accreditation standards and industrial instruments.
Ensure accurate and timely documentation reporting and activity data in accordance with regulatory and organisational requirements.
Role-model and embed St Vincents Mission Vision and Values across all areas of responsibility.
Your Contribution
Registered Nurse with current AHPRA registration.
Minimum 5 years postgraduate nursing experience.
Minimum 2 years nursing leadership or management experience in an acute sub-acute emergency community home-based or virtual care setting.
Experience in Hospital in the Home virtual care or community-based service delivery is highly regarded.
Demonstrated experience in service establishment service development or leading change in complex healthcare environments.
Strong clinical governance operational and stakeholder engagement capability.
Proven ability to lead develop and motivate high-performing teams.
Current drivers licence and access to a reliable vehicle.
Successful completion and ongoing maintenance of Responsible Person screening checks.
