Job Summary

Nurse Care Manager

Behavioral Health Home programs

The position of Nurse Care Manager is a member of the Behavioral Health Home (BHH) Adult Service Team and the BHH Childrens Service both BHH programs the Nurse Care Manager is to provide nursing consultation to the BHH Care Coordinators and Peer/Parent Navigators oversight of BHH health outcomes and collaboration with BHH Primary Care Partners. It involves health data interpretation and reporting; clinical problem solving; and collaborating with an integrated health and wellness service team serving adults and children with serious and persistent mental illness. It supports recovery-oriented service that strives to improve health outcomes through health promotion & education and primary secondary and tertiary prevention; cost effectiveness; and improved consumer-provider partnership for the population served by the BHH.

This role fosters highly skilled communication between behavioral health staff peer/parent navigators primary care partners MaineCare staff and other members of the clients care team in the interest of consumers and families served at the BHH and CBHH.

Schedule: Full-time salaried role M F regular business hours.

Location: on-site in South Portland ME

Qualifications:

  • Registered Nurse holding a valid license to practice nursing in the State of Maine the license must be in good standing; with two (2) of experience required.
  • Experience with and knowledge of people experiencing mental health and substance use issues poverty homelessness chronic disease domestic violence trauma and/or oppression very preferred.
  • Knowledge of commitment to or proficiency in recovery-based harm reduction trauma-informed and strengths-based approaches preferred.
  • Familiarity with utilizing and reporting health population data.
  • Experience with integrated healthcare for adults is preferred but not required.
  • Experience in electronic data collection and reporting is very helpful. Comfort and experience with ECR is critical.
  • Appropriate licensing/credentialing to meet all MaineCare BHH/CBHH Regulations.
  • Computer proficiency in Microsoft Office is a requirement.
  • Must have a professional Nursing license that is current and in good standing.
  • Must be able to successfully pass a criminal background child protective service check & sex offender check.
  • Must not be on the state or federal suspension and disbarment list.
  • Normal office conditions such as sitting with occasional standing walking reaching stooping bending kneeling crouching typing and lifting to 10 pounds.
  • Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to the agencys location to complete work. A good driving record is required.
