Behavioral Health Home Programs

The Opportunity Alliance is seeking a Nurse Care Manager to join our team-based care coordination (case management) model called Behavioral Health Home.

The Nurse Care Manager is an integral member of both the Behavioral Health Home (BHH) Adult and Childrens Service this role you will provide nursing consultation to BHH Care Coordinators and Peer/Parent Navigators oversee health outcomes and collaborate closely with BHH Primary Care Partners.

This position is part of an integrated health and wellness team serving adults and children with serious and persistent mental illness supporting recovery-oriented care that emphasizes health promotion education prevention across all levels cost-effective services and strong consumerprovider partnerships. The role combines data interpretation clinical consultation and short-term client engagement. It integrates clinical expertise with care coordination and requires strong skills in health data analysis and reporting clinical problem-solving and interdisciplinary collaboration.

This is an exciting opportunity to bridge physical and behavioral healthcare. You will play a central role in advancing a comprehensive integrated care modelparticularly for individuals with complex medical needs or those navigating critical transitions in care.

This role offers a flexible weekday schedule during standard business hoursno mandatory overtime or shift coverageallowing for a healthy work-life balance.

Key Responsibilities Include:

  • Providing nursing consultation and clinical guidance to BHH staff
  • Supporting clients in person remotely and assisting BHH staff as needed
  • Interpreting health data and contributing to reporting and outcome tracking
  • Collaborating on the development and implementation of policies protocols and best practices
  • Offering consultation to support clients transitioning from hospitals emergency departments and care facilities

Schedule: Full-time salaried role M F regular business hours.

Location: Hybrid and on-site in South Portland ME

Qualifications:

  • Registered Nurse holding a valid license to practice nursing in the State of Maine the license must be in good standing; with two (2) of experience required.
  • Experience with and knowledge of people experiencing mental health and substance use issues poverty homelessness chronic disease domestic violence trauma and/or oppression very preferred.
  • Knowledge of commitment to or proficiency in recovery-based harm reduction trauma-informed and strengths-based approaches preferred.
  • Familiarity with utilizing and reporting health population data.
  • Experience with integrated healthcare for adults is preferred but not required.
  • Experience in electronic data collection and reporting is very helpful. Comfort and experience with ECR is critical.
  • Appropriate licensing/credentialing to meet all MaineCare BHH/CBHH Regulations.
  • Computer proficiency in Microsoft Office is a requirement.
  • Must have a professional Nursing license that is current and in good standing.
  • Must be able to successfully pass a criminal background child protective service check & sex offender check.
  • Must not be on the state or federal suspension and disbarment list.
  • Normal office conditions such as sitting with occasional standing walking reaching stooping bending kneeling crouching typing and lifting to 10 pounds.
  • Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to the agencys location to complete work. A good driving record is required.
