Preferred Qualifications

Recent Case Management experience in an acute care hospital setting

Experience with Kaiser Permanente systems workflows or member population

Job Description RN Case Manager
RN Case Manager is responsible for coordinating and managing the care of hospitalized patients to ensure safe timely and cost-effective outcomes. This role focuses on care progression discharge planning utilization review and interdisciplinary collaboration to support patients throughout their hospital stay and transition to the next level of care.
The Case Manager works closely with physicians bedside nurses social workers and external providers to ensure appropriate resource utilization regulatory compliance and continuity of care. The RN serves as a liaison between patients families healthcare teams and post-acute services.
Duties & Responsibilities

Coordinate and oversee care progression for assigned patients to ensure safe efficient and timely treatment.

Perform initial and ongoing assessments to identify discharge needs barriers and appropriate level of care.

Develop and implement comprehensive discharge plans in collaboration with patients families and interdisciplinary teams.

Participate in daily rounds/huddles communicating updates and identifying risks or delays.
-
Coordinate transfers to appropriate settings such as SNF rehab LTAC home health or outpatient services.
-
Obtain and track insurance authorizations referrals and approvals for procedures services and post-acute care.

Monitor patient progress length of stay and resource utilization to support quality outcomes and cost-effective care.

Provide education and support to patients and families regarding care plans discharge instructions and follow-up needs.

Maintain accurate documentation of assessments care coordination activities and discharge plans in the electronic health record (EHR).

Collaborate with utilization review social services and clinical teams to ensure compliance with regulatory and payer requirements.

Serve as a liaison to ensure smooth transitions across the continuum of care and help reduce readmissions.
