About the Opportunity
Star Nursing is seeking an experienced Director of Care Management to lead hospital-based care coordination and utilization management programs.
This leadership role oversees care management operations across multiple facilities ensuring patients receive appropriate care coordination from admission through discharge. The Director will guide interdisciplinary teams improve care transitions and support efficient use of healthcare resources while maintaining high-quality patient outcomes.
Position Summary
The Director of Care Management provides strategic leadership for care coordination teams responsible for discharge planning utilization review and patient advocacy. This role ensures effective communication among providers clinical teams and external agencies while promoting efficient care transitions and patient-centered outcomes.
Key Responsibilities
Care Management Leadership
- Direct and coordinate care management processes across hospital departments
- Lead interdisciplinary teams to ensure effective patient care coordination
- Support seamless transitions from admission through discharge planning
- Advocate for patient-centered care and resource optimization
Operational Oversight
- Monitor care management workflows and operational performance
- Ensure compliance with regulatory requirements and healthcare standards
- Oversee utilization review processes and resource management
- Participate in departmental goal setting and strategic planning
Team Leadership & Development
- Recruit train and supervise care management staff
- Promote professional development and staff performance improvement
- Foster a collaborative and supportive team environment
Collaboration & Communication
- Maintain strong relationships with physicians and clinical teams
- Coordinate with external referral agencies to facilitate timely discharges
- Lead utilization management initiatives and performance review processes
Minimum Qualifications
- Bachelors Degree in Nursing required
- Masters Degree preferred
- Active RN license
- Minimum 2 years acute care clinical experience
- Minimum 3 years leadership experience in care coordination or case management
- Strong knowledge of utilization review and discharge planning processes
Preferred Qualifications
- Experience leading hospital-based case management teams
- Knowledge of utilization management and payer regulations
- Strong leadership communication and problem-solving skills
- Experience managing multidisciplinary care coordination teams
