Job Title: Licensed Practical Nurse (LPN)
Job Location: Newark NY 14513
Shift: Mid Shift 5x8-hour shifts (2:00 PM 10:00 PM)
Assignment Duration: 13 weeks (high possibility of extension)

Position Summary:
The Licensed Practical Nurse (LPN) provides direct patient care in a behavioral health and skilled nursing environment. This role requires experience in behavior management patient monitoring and acute care support with the ability to function independently with minimal supervision.
Key Responsibilities:
Perform the majority of patient care functions in an acute care setting with minimal orientation and supervision.
Greet patients direct them to exam rooms and prepare them for examinations.
Assist during routine exams including setting up trays for injections measuring and recording patient progress and taking vital signs.
Assist with general office duties reception appointment scheduling filing and chart preparation.
Contact insurance companies to obtain authorizations for procedures and treatments.
Contact primary care physicians to obtain new referrals or referral extensions.
Assist physicians in patient care activities.
Maintain exam rooms and clinical areas.
Administer injections and assist with Pap smears and other procedures.
Implement nursing care activities as outlined in nursing care plans.
Observe and report patient responses to treatments.
Participate in nursing unit management activities.
Maintain accurate documentation within the electronic medical record system.
Adhere to infection control isolation precautions and safety protocols.
Required Skills:
Experience in a locked unit and skilled nursing facility/nursing home setting.
Behavior management techniques and managing assaultive behavior.
Blood glucose monitoring and testing.
De-escalation protocols.
Intake and output monitoring.
Isolation precautions.
Specimen labeling.
Vital sign monitoring.
Experience using Electronic Medical Records (strongly desired).
