Job Summary
Hospice Bereavement Coordinator
Salary: $61-66k/yr BOE
Schedule: M-F 8:00 AM-5:00 PM
Location: Blue Ash Ohio and the surrounding area
Requirements of the Bereavement Coordinator role:
Associate degree or higher in a related field
Experience with counseling individuals and/or groups
Valid Drivers License and Auto insurance Coverage
Reliable transportation
About the Bereavement Coordinator role:
Responsible for planning implementing and maintaining a bereavement program that supports the families/caregivers of the deceased for up to one (1) year after the patients departure.
Development and implementation of the bereavement plan of care for the bereaved following the hospice patients death.
Develop a bereavement plan of care using the bereavement assessment the risk assessment input from the IDG and interaction with the bereaved following the patients departure.
Provide bereavement support to hospice personnel coping with work-related grief.
Provide memorial services grief support groups and community education send mailings bereavement calls bereavement visits and other services deemed necessary.
