Job Summary and Responsibilities
As a Patient Placement Coordinator you will provide critical support and meticulous coordination ensuring the efficient and appropriate placement of patients across our healthcare facilities to optimize patient flow and resource utilization.
Every day you will expertly assess patient needs based on clinical criteria communicate with admissions and unit staff manage bed availability and meticulously facilitate transfers ensuring timely access to the right level of care.
To be successful you will demonstrate outstanding organizational skills strong communication abilities and a proactive problem-solving demeanor contributing significantly to operational efficiency and patient safety.
- Performs duties according to department specific expectations.
- Identify the care and resources required for accessing hospital services for surgical procedural non-invasive diagnostic invasive diagnostic admitted and clinical procedure patients.
- Provides timely accurate and complete medical record documentation and data collection initiating the electronic medical record and patient assessment clinical documentation.
- Establishes positive working relationships with physicians and their office staff with a focus on customer service and ease of access to hospital services.
- Provides patient information preparation and education for impending surgery tests procedures admissions through the patient interview. Identifies patient/care giver learning needs and initiates and provides for age appropriate education.
- Effectively functions as a concierge for patients families physicians office staff and the patient care team. Promptly investigates and resolves problems or concerns or refers them to a higher authority.
