Position Purpose:
In accordance withthe Mission and GuidingBehaviors;theLicensed Practical Nurse - with Meds within the scope of the Ohio Nurse Practice Act will provide care in an atmosphere sensitive to each persons physical emotional social and spiritual needs.Functioning in the spirit of teamwork under the direction of a Registered Nurse is accountable for the delivery of patient care utilizing the Nursing Process within the standards policies procedures and guidelines of the Organization.

Relationship-based Care: Creates a caring and healing environment that keeps the patient and family at the center of care throughout their experience at Mount Carmel following the principles of our interdisciplinary care delivery system.
Provides nursing care ensures an environment of patient safety promotes evidence-based practice and quality initiatives andexhibitsprofessionalism in nursing practice within the model of the ANCC Magnet Recognition Program.
DELEGATION: Directs unlicensed staff in work related activities under the supervision of a Registered Nurse. Demonstrates the ability to managethe patient assignmenteffectively.
COMMUNICATION: Communicates pertinent clinical information toappropriate membersof the patient care team.Keepspatient/family appraised/updated.
EDUCATION: Provides patient/family education under the direction of the Registered Nurse
CONTINUITY PLANNING: In conjunction with the physician and RNidentifiesdischarge needsparticipatesinplanningand completes continuity of care forms asappropriate.
COLLABORATION: Collaborates with Providers house staff Registered Nurses and other health care disciplinesregardingpatient care.
SAFE PRACTICE: Performs treatments and procedures according to approved standards policies procedures and guidelines. Verifies and implements physician/other licensed independent practitioners orders. Administers medications according to theorganizations standards policiesproceduresand/or guidelines.Initiates appropriate emergency procedures within the scope of practice under the direction of the RN/physician.Practicesproper patient identification procedures. Maintains equipmentappropriate tothe care environment. Establishes priorities for the delivery of nursing care and meets patient needs.
DOCUMENTATION: Documents accurately and completely pertinent information/datain accordance withdocumentation standards policiesproceduresand/or guidelines.
COMPUTER SKILLS:Utilizescomputer systems effectively including. ATK. Systemsappropriate toarea. Registrar. Appropriate use ofInternet/Intranet.
INITIATIVE: Takes responsibility tofacilitateimprovements in patient services staff/physicianrelationshipsand the working climate. Is responsible/accountable for professional development andmaintaininglicensure requirements.Takesresponsibility forowneducation.

Minimum Qualifications:
Education: Graduate of School of Practical Nursing
Licensure / Certification: Current license to practice as a Licensed Practical Nurse (LPN) in the State of Ohio.
Successful completion of an LPN IV therapy course with proof of certification through one of the following: LPN curriculum (i.e.part of Ohios required curriculum National certification (i.e.NAPNES) or through IV training course that includes pharmacology at a hospital.
