Life Without Barriers Synergy Youth and Family Mental Health Service provides intensive support to young people aged 12 to 25 who are experiencing severe and complex mental health challenges. Our team of clinicians and case managers provides comprehensive clinical assessment and formulation.
Support is offered in community settings such as the young persons home or other local community environments allowing therapeutic work to take place in real time and within the context of their daily life. The service also helps young people develop their functional skills and daily living abilities while working closely with them and their support network for up to 12 months.
Role: Clinician (Mental Health)

Office Location: Hobart including opportunity for community and outreach work
In this role you will work one-on-one with young people and their families supporting them to improve their mental health well-being and overall functioning while walking alongside them as they achieve their goals.
As part of a skilled supportive wrap-around outreach mental health team you will use your expertise to conduct assessments develop care plans and deliver therapeutic interventions and case management. You will provide outcome-focused support to young people and families with complex needs helping them achieve meaningful positive results.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Tasmanian Working with Vulnerable People Registration.
