Position Details:
Hartford Hospital is seeking a Physician Assistant or Nurse Practitioner to join their Department of Medicine team! We are looking for a non-clinical APP to conduct outreach post discharge calls to complex patients ensuring discharge instructions are adhered to. Go over medication reconciliation lists and confirm prescriptions are filled and picked up. Establish follow up with PCP/specialists.
