Full-Time Outpatient Community Health
Position Overview
We are seeking a dedicated and patient-centered Nurse Practitioner or Physician Assistant to provide high-quality outpatient primary care within a community health clinic environment. This position offers strong clinical collaboration excellent provider support and the opportunity to serve diverse and medically underserved patient populations.

Key Responsibilities

Provide comprehensive primary care services for patients of all ages

Conduct physical exams health assessments and diagnostic evaluations

Diagnose treat and manage acute and chronic conditions
-
Prescribe medications and develop treatment plans within scope of practice

Coordinate patient care referrals and follow-up services
-
Maintain accurate timely and compliant medical documentation
-
Collaborate closely with physicians nursing staff and care teams
Qualifications
-
Active and unrestricted California NP or PA license
-
Board certification (NP or NCCPA)
-
Current BLS certification and active DEA registration
-
12 years of clinical experience preferred
-
Experience in community health is a plus
-
Bilingual skills preferred
Compensation
$131040 $170352 annually (based on experience)
