SH-855 Nurse Practitioner New OrleansLA

Position Overview:
Apply clinical skills to the management of healthcare for medically complex older adults working collaboratively with an interdisciplinary team to address participants medical emotional and social well-being. The Nurse Practitioner must be able to perform the essential functions of the job with or without reasonable accommodation.

Key Responsibilities

  • As a member of the PACE Interdisciplinary Team (IDT) prepare and update comprehensive Care Plans for participants that include medical emotional and psychosocial aspects of care.
  • Remain attentive and responsive to input from team members participants and caregivers regarding participant health and communicate important clinical information to the IDT on an ongoing basis.
  • Conduct comprehensive history and physical examinations for new participants to assist in preparation of a comprehensive Care Plan.
  • Reassess participants medical conditions at least every six months as part of the semi-annual assessment for updating the Care Plan.
  • Manage participants medical care for both acute and chronic issues.
  • Review outside clinical information for participants including specialty clinic appointments hospitalizations and nursing home stays.
  • Document changes in a participants condition in the medical record in a timely manner consistent with documentation policies established by the Medical Director.
  • Work in the PACE clinic at either of the two PACE centers to see participants as scheduled and as needed.
  • Provide in-home care as needed.
  • Contact and inform caregivers of any medical changes and return calls from participants caregivers and staff in a courteous and timely manner.
  • Serve as the provider on-call for the on-call nurses during evenings and weekends returning calls in a courteous and timely manner.

Qualifications & Requirements

  • Education: Nursing Degree from an accredited university and a graduate of a certified or accredited Nurse Practitioner Program; or Masters Degree in Clinical Nursing with five years nursing experience.
  • Licensure: Must have a current license as an Advanced Practice Registered Nurse (APRN).
  • Experience: Preferred five-to-seven years nursing experience with three years of experience as a Nurse Practitioner.
  • Skills: Strong clinical assessment and management skills for medically complex older adults; ability to work effectively within an interdisciplinary team; excellent verbal and written communication abilities.
  • Compliance: Must adhere to all applicable state and federal regulations and organizational standards.

Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Sitting standing walking talking. Must be able to lift thirty pounds at times. Physical Demands: Sedentary physical activity performing non-strenuous daily activities of an administrative nature.
