PRIMARY FUNCTIONS
The Immunization Action Plan Program Manager leads the City of New Haven Health Departments efforts to prevent vaccine-preventable diseases through strategic planning implementation and evaluation of community immunization initiatives. The position manages all aspects of the Immunization Action Plan ensuring compliance with local state and federal public health standards while advancing equitable access to vaccines.
Under the direction of the Health Programs Director the Program Manager supervises outreach staff monitors program performance and fosters partnerships with healthcare providers schools and community organizations. Serving as the departments subject-matter expert on immunization policy and practice the incumbent promotes data-driven decision-making coordinates education and outreach efforts and strengthens public confidence in immunization as a cornerstone of community health.
TYPICAL DUTIES AND RESPONSIBILITIES
- Oversees the daily operations of the Immunization Action Plan including strategic planning implementation monitoring and evaluation of program goals and objectives to enhance access to and use of services to prevent communicable diseases.
- Supervises program staff. Onboards trains oversees and supports their work.
- Provides ongoing surveillance of program productivity and accomplishments. Develops and maintains detailed work plans timelines and performance metrics for program activities.
- Prepares and submits required programmatic financial and performance reports as required to local state and federal partners.
- Ensures compliance with City State of Connecticut Department of Public Health (DPH) and Centers for Disease Control and Prevention (CDC) standards policies and reporting requirements.
- Assists with researching and applying for new funding opportunities.
- Builds and sustains partnerships with hospitals community-based health organizations private practices and schools to strengthen immunization efforts.
- Convenes staffs and facilitates the Immunization Advisory Council ensuring active stakeholder participation and collaboration.
- Coordinates interagency efforts to improve access to vaccines particularly among medically underserved and high-risk populations.
- Oversees the coordination of vaccine delivery systems including assessments accountability outreach referrals and induction.
- Coordinates transfers of vaccinations among providers in service area as needed.
- Oversees provider site visits assessments and audits to providers to ensure compliance with immunization standards and best practices.
- Manages and trains staff and providers to on the use of the states immunization record and data reporting systems as needed.
- Analyzes immunization data to identify coverage gaps trends and areas for targeted outreach or intervention; Develops and maintains a referral system to identify children who do not have a medical home.
- Conducts outreach and follows up with children enrolled in the states immunization record system to connect them with providers and update their immunization records as needed.
- Develops culturally and linguistically appropriate educational materials and campaigns (e.g. pamphlets brochures social media posts) to increase vaccine awareness and acceptance.
- Plans promotes and conducts community events health fairs and educational sessions on vaccine-preventable diseases and immunization best practices; Hosts virtual and in-person presentations and educational opportunities that address vaccine preventable diseases.
- Serves as a primary resource for healthcare providers and the public on immunization policy practices guidelines and updates.
- Plans events and promotional campaigns around national health observances.
- Collaborates on emergency preparedness and response activities related to infectious disease outbreaks.
- Performs related duties as required.
EDUCATION QUALIFICATIONS & EXPERIENCE
Possession of a masters degree with a major in Public Health Community Health Nursing Public Administration or a related field. Three to five years professional experience working directly in a community-based agency with evidence of high performance management experience strong interpersonal skills and writing capabilities. Bilingual English/Spanish lieu of a masters degree an additional 5 years of closely related experience may be substituted.
KNOWLEDGE EXPERIENCE SKILLS & ABILITIES
- Knowledge of the principles practices and regulations applied in public health as well as current developments in the fields of community health and immunizations.
- Knowledge of the cause transmission and prevention of vaccine preventable diseases.
- Knowledge of how to plan and evaluate community health and/or social service projects including development of goals objectives and evaluation criteria.
- Proficiency with Microsoft Office applications such as Excel and Word for data entry and for creating various reports spreadsheets and presentations.
- Competency in using email clients (e.g. Outlook Gmail) and calendar applications for scheduling and managing correspondence.
- Ability to develop concise records and prepare financial and programmatic reports.
- Ability to analyze data and apply findings to improve program effectiveness and inform decision-making.
- Experience supervising staff and managing daily expectations; Ability to develop and monitor staff work plans and ensure performance accountability.
- Must be self-motivated organized and able to work independently and as part of a team.
