About the Program:The Family Based Services program is an intensive team approach to community based services engaging individuals with complex needs aimed to support the client in remaining in the least restrictive living environment restore adaptive functioning in the home and strengthen the use of coping strategies and support system. This program targets families with DCFS involvement and/or youth on formal probation.
Role Summary:We are currently seeking a dedicated Mental Health Services Provider to deliver comprehensive mental health services ensuring exceptional client care. As a vital member of our interdisciplinary team you will contribute to creating a healthy therapeutic and safe environment for clients. Responsibilities include facilitating intensive family-centered and community-based services for children adolescents and their families. This position serves as the Lead of the Child and Family Team with a commitment to confidentiality regulatory compliance and professional growth you will play a key role in promoting the well-being of the clients we serve. This Facilitator position will provide services to community based clients for our Wraparound Approach Wraparound program is an intensive team approach to community based services engaging individuals with complex needs aimed to support the client in remaining in the least restrictive living environment restore adaptive functioning in the home and strengthen the use of coping strategies and support system. This program targets families with DCFS involvement and/or youth on formal probation.
Clinical Supervision is provided with ability to earn all clinical hours towards licensure
This position does qualify towards Public Student Loan Forgiveness as a non-profit organization
Duties and Responsibilities:
- Perform initial and ongoing strengths-based assessments of children and families. Identify and prioritize familys needs across all domains.
- Assist with the development of a Child and Family Team (CFT) which includes multi-disciplinary professionals the child and family extended family members and informal community supports. Participate in weekly CFT meetings.
- Facilitate team meetings and fully involve families in the planning and decision-making process.
- Demonstrate leadership of the Wraparound Team and facilitates communication and collaboration between team members clients and outside service providers.
- Assist in the development of a comprehensive plan of care ensuring strategies interventions and services are family-centered flexible and community-based.
- Ensure collaboration and coordination among all agencies involved in the implementation of the families plan.
- Interface with Department of Children and Family Services Department of Probation Department of Mental Health court systems law enforcement schools health care providers and other community liaisons.
- Maintain weekly service provision requirements.
- Assist family in the incorporation of informal supports natural family helpers and other community resources.
- Ensure familys development of family-centered help-giver skills and the evaluation of contributions to positive outcomes for children and families.
Qualifications
- Masters degree in social work Psychology or Counseling is required
- Active registration or licensure with California BBS is required
- Experience in In-Home Services or work with highrisk children adolescents and their families required
- Strong clinical skills in individual family play and group therapy are required
- Good oral written and organizational skills are required
- Must possess and maintain a valid California Drivers License and a driving record acceptable to the agencys insurance carrier.
- Bilingual and bicultural skills in Spanish required
- Previous Wraparound Approach Services Program experience preferred
