Position Overview:

The Family Nurse Practitioner provides leadership and supervision for the programs evidence based treatment plan for Hepatitis C and co-occurring disorders. The Medical provider will deliver patient-centered Hepatitis C prevention diagnosis and treatment. Manage Co-occurring conditions while coordinating care across interdisciplinary teams to provide whole person trauma-informed care tailored to underserved populations. This role includes managing Hepatitis C cases prescribing antiviral therapies and monitoring treatment outcomes. The provider will take part in addressing co-occurring mental health and substance use disorder through behavioral health screenings integrated care planning and appropriate consultation and referrals. Provide integrated low barrier HCV HIV SUD and SMI treatments in communities that are at increased risk of HCV.

Participate in NARA NW Hep C Free Initiative an integrated care approach to prevent test for treat and cure HCV in individuals with SUD and/or SMI particularly in communities disproportionately affected by homelessness. This program supports low barrier integrated substance use prevention SUD and SMI treatment and recovery supports and HCV/HIV prevention testing and access to treatment among individuals with or at risk for HCV with or without HIV. Given the crisis of homelessness in the U.S. this program looks to especially reach communities that have been heavily impacted by the intersection of homelessness substance use/substance use disorder and mental illness and infectious diseases specifically HCV.

What you will do:

  • Provides comprehensive whole person medical care services with a focus on the diagnosis treatment and management of Hepatitis C and related health conditions. Related health conditions include Mental Health Psychiatric and Substance Use Disorders.
  • Conducts initial and ongoing health risk assessments including screening for substance use HIV and other high-risk behaviors or conditions.
  • For people who test positive for HCV RNA provide on-site direct-acting antiviral-based treatment peer support and care coordination.
  • Develops and documents a plan of care with treatment goals and outcome evaluations in accordance with clinical standards.
  • Assess clients to determine who would benefit from in person or virtual Directly Observed Therapy (DOT) for HCV.
  • Coordinates care with internal teams and external partners including Medical Behavioral/Mental Health Pharmacy Obstetric providers Hospitals and Laboratories.
  • Follow-up to ensure individuals are engaged and retained in care and treatment and able to complete their HCV treatment.
  • Assists in navigation to prenatal care for chemically dependent pregnant patients through consultation and referral to obstetric providers.
  • Offers patient education on topics such as birth control safer sex and the health consequences of high - risk behaviors.
  • Encourages appropriate family involvement in patient care in alignment with patient preferences and care team recommendations.
  • Serves as a patient advocate and facilitates access to specialty and support services helping patients navigate complex healthcare systems.
  • Collaborates with interdisciplinary team members including medical providers psychiatric providers nurses case managers and outreach staff to ensure whole person care.
  • Participates in mobile outreach in an outdoor environment and in clinic-based services to increase access for underserved and high-risk populations.


Qualifications

We would like to hear from people that have:

  • Licensed as a Nurse Practitioner in the state of Oregon.
  • Experience in working in Primary care clinic settings performing medical assessments developing and implementing patient plans of care.
  • Evidence of experience sufficient for successful performance of duties noted above.
  • Ability to maintain professional boundaries with clients the community and other staff members.
  • Experience working with individuals affected by substance use homelessness or other complex social needs is strongly preferred
  • Work collaboratively with clients other NARA staff team members and to treat everyone with respect and dignity at all times.
  • Experience working within the Native American/Alaska Native community is considered a plus.
  • Culturally responsive and trauma-informed care approach required
  • Proficient in Electronic Health Records (EHR) systems and documentation standards
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass criminal background and DMV checks.
