Job Summary
JOB PURPOSE:The Case Managerfor Eagle Lake Childrens Centeris responsible for assessing the needs of unaccompanied minor children in care developing Individual Service Plans screening for Human Trafficking concernsfacilitating the safe and timely release or discharge of children and youth and documenting the provision of services in the childs case Case Manager will work primarily to help reunify unaccompanied minor children who are placed at the Eagle LakeChildrens Centerby the Office of Refugee and Resettlement (ORR).
Qualifications
ESSENTIAL JOB RESPONSIBILITIES:
- Oversees all aspects of the Unaccompanied Childs (UC) case and provides releaserecommendationfor safe andtimelyrelease.
- Maintains a caseload ofup to8clients per ORR ratio.
- Maintain case file documentationin accordance withORR requirements.
- Maintains confidentiality when gathering documentingconsultingorconsolidatinginformation necessary for the case management assessment and the Individual Service plan of care.
- Maintain regular contact with each UC UCs family when possible andestablishservice providers that will provide services to the child upon release.
- Gathers historical and current datarelativeto completion of a comprehensive case management assessment including but not limited to school records medical records mental health treatment legal status and court involvement social and family strengths and needs support systems and specific cultural issues.
- Presents case and case updates to federal stakeholders on a weekly basis.
- Participates in treatment team meetings staff in-servicetrainingand department meetings.
- Completes Significant Incident Reports (SIRs) per federal policy requirements.
- Conducts internal chart reviews tomonitordocumentation compliance.
- Other duties as assigned.
ADDITIONAL QUALIFICATIONS/REQUIREMENTS
- Mustbeat least21 years old.
- Proficiencyin Spanish/English (written and spoken language skills)required.
- Intermediateproficiencyin Microsoft Office products and Google tools.
- Successfullypass a TB test annually.
- Successfully pass a drug screen.
- Be physically mentally and emotionally capable of performing assigned tasks and have the skills necessary to perform assignedtask.
- Pass a criminal history screen including state federaland local child protection agency registries.
- Ability to work independently and exercisea high levelof confidentiality.
- Affidavitfor Applicants for Employment with a Licensed Operation or Registered Childrens Home.
Required PositionQualifications:(as required by federal contract)
- Bachelors degree ina behavioralscience humanservicesor social services field.
- ORR Case Management experiencedpreferred.
LICENSES:Drivers License with clear recordrequired.
WORK LOCATION:Eagle LakeChildrens Centerin-person.
OTHER:Must be available and willing to travel in case ofan emergencyevacuation and as the businessneeddictates. Must be available and willing to work nights weekends and holidays asrequiredto meet business needs.
