POSITION PROFILE
The Program and Quality Facilitator is a non-clinical role responsible for supporting quality improvement and operational excellence across all organizational programs while providing dedicated support to the care coordination and care management team within the Integrated Care Program. This position ensures compliance with standards monitors operational metrics facilitates audits and supports process improvement initiatives to enhance service delivery and patient outcomes.

Key Responsibilities
Program Coordination
- Supports the day-to-day operations for the Integrated Care Program ensuring timely and efficient service delivery.
- Act as a liaison between interdisciplinary teams administrative staff and external partners to maintain seamless coordination and maximized client outcomes.
- Provides contingency support for client service coordination.
Quality and Compliance
- Plan conduct and document quality audits across all programs to ensure adherence to organizational standards and regulatory requirements.
- Identify gaps and recommend corrective actions based on audit findings.
- Monitors service performance including coordination and care management metrics and actions or escalates as required.
Quality Service Checks
- Conduct regular quality service checks across programs to verify compliance with established standards.
- Action findings promptly by initiating corrective measures and escalating issues as required.
Process Improvement Initiatives
- Identifies and supports implementation of continuous improvement projects across programs focusing on efficiency quality and client-centered care.
- Utilize data analysis and stakeholder feedback to implement evidence-based changes.
Billing & Financial Accuracy
- Support accurate and timely billing for services provided under the Integrated Care Program.
- Verify documentation and service records to ensure compliance with funding and reimbursement guidelines.
- Collaborate with finance teams to resolve discrepancies and maintain audit-ready billing processes.
Collaboration & Facilitation
- In collaboration with the leadership team organizes and lead workshops team meetings and improvement sessions to foster collaboration and alignment on quality goals.
- Promote best practices and support staff education on program guidelines and quality standards.
Reporting & Documentation
- Prepare comprehensive reports on audit results quality metrics and improvement activities for leadership review.
- Maintain accurate documentation of processes and compliance activities.

QUALIFICATIONS
- Post-secondary education in health administration business or a related field or equivalent experience in a home health setting.
- 3 years of experience in program coordination quality improvement healthcare operations (non-clinical) and Ontario Health atHome billing practices.
- Strong knowledge of quality assurance auditing and process improvement methodologies.
- Excellent facilitation communication and organizational skills.
- Proficiency in data analysis and reporting tools.
