About the Role
Help shape equitable health outcomes for Quota Refugees during resettlement.
Tū Ora Compass Health is seeking two experienced and compassionate Clinical Case Managers to join our Refugee Health Service team. One position will be based in the Wairarapa and the other in the Hutt Valley. Our ideal candidate in the Hutt Valley will hold a Community Nurse Prescriber qualification.
The purpose of this role is to provide proactive culturally responsive primary care services to Quota Refugees during their resettlement period. You will work closely with the Cross-Cultural Health Navigator and local Primary Care providers to ensure tailored health interventions that meet the unique needs of this community.
What Youll Do
As a Clinical Case Manager you will:
Deliver screening assessment management and treatment for acute and long-term conditions.
Support immunisation catch-up disease prevention and health education.
Coordinate care and connect individuals with other health and social services.
Focus on reducing avoidable hospital admissions through early intervention and continuity of care.
This work aligns with Goal 3 of the NZ Refugee Resettlement Strategy: Health and wellbeing: refugees and their families enjoy healthy safe and independent lives.
What Youll Bring
NZ registered nurse with 5 years clinical experience.
A Community Nurse Prescriber (ideal)
Passionate about equity and culturally safe care.
Experienced in working with diverse communities and multidisciplinary teams.
Flexible empathetic and proactive in problem-solving.
