Job Title: Virtual Care Coordinator
Location: New York NY
Duration: 13-Week Contract
Shift: Evening 3:00 PM 11:00 PM
Pay Rate: $24 $28 per hour
Position Summary
The Virtual Care Coordinator plays a key role in supporting virtual patient care and remote monitoring initiatives. This position focuses on patient safety clinical quality and operational excellence through telehealth services. The role requires strong collaboration with clinical teams leadership digital technology partners and telehealth vendors to ensure effective delivery of virtual care programs and positive patient outcomes.
Key Responsibilities
Telehealth & Virtual Care Operations
Support virtual patient observation and remote monitoring for safety and clinical needs
Triage and assign patients for virtual monitoring and observation
Monitor telehealth consult orders scheduled consults and follow-up virtual visits
Ensure telehealth sessions begin on time and are completed without technical issues
Provide continuous observation of assigned patients to detect changes in behavior or clinical status
Perform early warning monitoring (e.g. vital signs central line days Foley catheter days)
Document patient behavior interventions and escalations accurately
Quality Safety & Program Development
Conduct quality assurance activities and peer reviews of virtual care programs
Track analyze and report telehealth performance metrics and utilization data
Identify trends and assist with corrective action plans
Participate in case reviews and monitor outcomes
Collaborate with leadership on telehealth policies workflows and best practices
Submit utilization and assignment reports at the end of each shift
Training & Collaboration
Provide training to new staff on telehealth workflows platforms and documentation
Serve as liaison between physicians nursing staff technology partners and vendors
Offer real-time white glove technical support to clinical teams
Assist with interpretation services using approved methods as needed
Communication & Patient Support
Greet patients families and visitors and explain the virtual care role
Promptly respond to calls from clinical teams
Communicate concerns regarding patient safety or clinical changes immediately
Maintain patient privacy in compliance with HIPAA and information security policies
Operational & Administrative
Maintain accurate records of interventions and safety events
Attend staff meetings team briefings and review work communications regularly
Maintain attendance punctuality and a safe clean work environment
Required Skills & Competencies
Strong interpersonal and communication skills
Ability to navigate multiple technology platforms simultaneously
Critical thinking prioritization and problem-solving abilities
Flexibility and sound judgment in high-pressure situations
Proficiency in written and spoken English
Experience Requirements
Minimum 3 years of direct patient care experience including:
Telehealth or remote patient monitoring experience
Quality improvement outcomes tracking and data collection
Development of telehealth policies workflows or protocols
Use of electronic medical records (EMR/EHR)
Identifying and escalating changes in patient clinical status or safety risks
Collaboration with interdisciplinary healthcare teams
Understanding regulatory legislative and organizational telehealth considerations
