TheIntakeCoordinator administratively reports to the Director of the Mental Health Center and executes responsibilities under the supervision of the Department of Psychiatry the performance of all duties theIntakeCoordinator complies with the Illinois Mental Health & Developmental Disabilities Code hospital policies procedures and by-laws and reflects the philosophy mission and values of the institution.
These duties are to be performed in a highly confidential manner following the mission values and behaviors of Hospital and Medical Center. Employees are further expected to provide a high quality of care service and kindness toward all patients staff physicians volunteers and guests.
Job Responsibilities:Screening Assessment and Referral:
Screens incoming telephone calls and walk-in patients to identify service requests the urgency of need patient resources and appropriateness of Insight Mental Health Center as a provider and the level of care required. Refers to other levels of care in Behavioral Health Services when appropriate.
Provides resources contact information and active linkage when possible for needed services not provided by Insight Behavioral Health Services.
Case Management: Verifies medical coverage for all levels of outpatient mental health service obtains required initial insurance authorization for Mental Health Center outpatients and secures relevant clinical information from referring agencies/facilities physicians or patients seeking outpatient aftercare.
Liaison to Crisis Worker:Provides relevant patient information to the crisis worker for patients referred to the ED for admission and accepts referrals of deflected patients deemed not in need of admission.
Requirements: - Must be a Registered Nurse

- Minimum of one year of clinical experience working on an inpatient and/or outpatient behavioral health service or a comparable setting.

- Knowledge of major mental disorders across the age continuum and levels of care.

- Understanding of developmental norms across the age continuum and ability to apply age-appropriate expectations when engaging patients/families to obtain relevant history and current level of functioning.

- Knowledge of substance abuse and dependency disorders across the age continuum and levels of care.

- Functional status screening skills to select the appropriate intensity of care or level of intervention required.

- Competence in the use of the electronic medical record to document and retrieve pertinent patient information and generate required reports.

- Excellent interpersonal skills are necessary to engage patients and families in the screening process and to develop collegial relationships with professionals within and outside of the organization.

- Communication skills to clearly and concisely transmit essential referral information to clinicians regardingintakerequests.

- Ability to prioritize work assignments organize and trackintakeflow meet turnaround time for call return and submit required reports.
