SUMMARY:Native American Community Clinic/ Red Lake Tribal Health Programs are seeking a Linkage to Recovery Care Specialistto join our team. Thepositionis responsible forcoordinating keyrecoveryactivities and resourcesforcommunity memberswhohavebeen recentlyreleased from Linkage to Recovery Care Specialist role willstrengthencarecollaborationwithin anintegrativeNACC careteam that includes medical behavioralhealthharmreduction and traditionalhealing tobettersupportcommunity membersin understandingoptions of care to address substance useandnavigationinaccessing recoveryresources as well as building relationships with reservations and systems tostrengthen a bridge to community care.
Ideal candidates will have experience working with Native communities a strong understanding of recovery pathways and a commitment to trauma-informed culturally responsive care.
DUTIES AND RESPONSIBILITIES
- Field and follow up on referrals from clinics jails hospitals treatment facilities and outreach teams to support community members with identifying and connecting to needs while building long-term relationships.
- Implementing motivational interviewing techniques to empower community members to take an active role in their recovery.
- Host weekly drop-in sessions for recently incarcerated participants providing basic need supplies and connection to recovery resources that address social determinants of health.
- Collaborate with NACC traditional healing and harm reduction team to create an integrated care team approach in addressing the recovery needs of community members accessing care through various avenues.
- Support harm reduction events as needed.
- Collaborate with the maternal child health NACC care team to support high-risk patients experiencing or having a history of problematic substance use.
- Advocacy and outreach efforts to effectively connect community members to care.
- Conduct community needs assessments surveys and coordinate town hall meetings to identify community barriers to care.
- Meet patients in hospitals and other care and community settings.
- Take a lead role in implementing chemical health grant objectives and outcomes.
REQUIRED QUALIFICATIONS
- High school diploma or equivalent.
- Experience working in public health or chemical health services.
- Willing to work in an outreach capacity as needed.
- Active Minnesota Drivers License and clean driving record.
- Work Experience in a non-for-profit and healthcare preferred.
- Ability to work in a diverse culture preferred.
- Experience working within the Native American community preferred.
- Excellent written and verbal communication skills.
- Excellent organizational skills.
- Demonstrated commitment to promoting a diverse inclusive and respectful workplace.
