Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members is critical.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- Minimum of 2 years of hospital home health or hospice care experience.
- Ortho and stroke experience.
- AHA BLS Provider CPR.
- Active Registered Nurse license in the state of working.
