Job Summary:
The Registered Nurse (RN) will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other members of the team are critical to this role.

Responsibilities:
- Providing skilled nursing care.
- Performing patient assessments.
- Administering medications.
- Creating and implementing treatment plans.
- Collaborating with the interdisciplinary team.
- Managing cases.
- Providing hospice care.
- Performing Phase I and Phase II Recovery duties.
- Assisting with Endo/Laparoscopic General Ortho Ortho/Spine and Ortho/Total joints procedures.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- ACLS certification.
- BLS certification.
- PALS certification.
- Minimum 1 year of experience.
Preferred Skills & Certifications:
- Experience in Phase I PACU and Phase II Recovery.
- Experience with Endo/Laparoscopic General Ortho Ortho/Spine and Ortho/Total joints procedures.
Special Considerations:
- Includes 2 weekend call shifts per month when asked for 4 shifts total (2 weekend and 2 weeknight).
Scheduling:
- 11:00 AM to 7:30 PM shifts.
- 5 to 8 hour shifts.
