Specialist clinics provide over 100000 non admitted patient appointments per year across 23 different medical and surgical specialties. Specialist Clinics provide a gateway to the hospital for patients requiring elective surgery and ongoing chronic disease management. Our major referral base is via General Practitioners from all areas of Victoria. This is a fast paced environment where teamwork is essential.
About the Role
An exiting opportunity is now available for a Case Coordinator can be a Registered Nurse or Enrolled Nurse to join the team on a Permanent Full Time basis 40 hours per week (part time will be considered).
The Case Coordinator under the direction of the Nurse Unit Manager is responsible for coordinating patients across the continuum of care from receipt of referral to discharge from clinic while ensuring compliance with the St Vincents Hospital Melbourne (SVHM) Specialist Clinics Access Policy. A key function of this role will be implementing a case coordination framework for patients referred for a Specialist Consultation.
Classifications for this role will be RN28 ($110614.40 per annum pro rata).
