Responsibilities will include:
- Assisting NDIS participants along with their families and/or carers to identify their needs through assessment and person-centred planning.
- Evaluating developing and delivering evidence-based support plans within a complex support space.
- Developing meaningful goals and practical strategies utilising a positive behaviour support framework and with a commitment to eliminate restrictive practices in the home and community.
- Ensuring individual plans reflect our NDIS participants goals passions and strengths as well as ensuring they get more out of life.
- Building the knowledge capacity and strengths of those who support participants in their lives - family carers friends etc.
- Acting as an advocate and role model for best practice.
- Working collaboratively within a multidisciplinary team.
Qualifications :
To be successful youll need:
- NDIS Commission suitability as a Behaviour Support Practitioner at Core or Proficient level.
- Degree qualification in an allied health field (e.g Psychology Social Work & Counselling).
- Strong report writing skills and attention to detail.
- Excellent time management and ability to manage a varied caseload.
- Genuine passion for the disability sector with a proactive and solution-focused approach.
