Were looking for a dedicated Licensed Practical Nurse (LPN) to join our team and make a meaningful impact by providing exceptional care and support to individuals in their healthcare journey through coordinated patient-centered medical care.

Key Responsibilities:

Care Team Support:

  • Provide support assistance and be a resource for Medical Assistants and nursing staff.
  • Orient new nursing staff to the Electronic Health Record and clinical policies and procedures providing ongoing support as needed.
  • Ensure nursing staff understand and comply with clinical policies collaborating with the ADON on training needs or consistent daily issues that need to be addressed.
  • Assist with direct patient care issues such as starting IVs and monitoring administration of medications.
  • Monitor the daily flow of assigned Care Team duties.

Communication & Coordination:

  • Coordinate daily Care Team huddles.
  • Facilitate communication with patients.
  • Coordinate patient group education courses based on patient requests and identified needs.
  • Participate in the interpretation and implementation of Care Team policies and procedures.
  • Coordinate patient exit visit (discharge) verifying preventative tests self-management and ensuring patient understanding of discharge instructions medical condition and need for follow through.
  • Manage and assign patient care tasks (phone calls messages labs etc.) during PCP absences to other health care team members.

Clinical Duties:

  • Complete clinical assessment of walk-in patients.
  • Complete task assignments as appropriate following prescription refill protocol and clinical protocols.
  • Review lab tracking reports daily to assure lab quality.
  • Manage all lab results according to nursing/clinical protocols.

Requirements:

  • Associate Degree in Nursing (ADN.)
  • Current LPN in Colorado.
  • Knowledge of equipment and technology used in the nursing area.
  • BLS certification/training and certification
