Diabetes Care Plus our expanding clinical service is a cornerstone of this mission. We support individuals and families living with diabetes and those at risk by providing high-quality care technology support and accessible education across Victoria.

YOUR IMPACT

Were seeking a forward-thinking Diabetes Nurse Practitioner to play a pivotal role in the growth of Diabetes Care Plus particularly in advancing our diabetes technology service.

In this role you will deliver advanced clinical care support people living with diabetes and their families and lead practice development in modern diabetes management. This includes assessment diagnosis prescribing education and care coordination.

A key focus will be establishing and strengthening referral pathways and relationships across the health system ensuring our services are accessible collaborative and responsive to community needs.

Key Responsibilities:

  • Deliver advanced diabetes clinical care through in-person consultations telehealth home/residential visits and group settings providing assessment management and prescribing within Nurse Practitioner scope (some travel and after-hours may be required).
  • Lead and expand Diabetes Victorias clinical service model working closely with the Clinical Services Coordinator to enhance systems pathways and the overall quality of care.
  • Provide mentorship and clinical leadership to clinicians within the service supporting capability building and high-quality practice.
  • Strengthen clinical governance by contributing to the development and review of clinical policies procedures guidelines and educational resources.
  • Provide expert diabetes advice and support across Diabetes Victoria including input into resources media requests (as appropriate) and membership or community engagement activities.
  • Maintain professional accreditation and contribute to continuous improvement including staying current with diabetes technologies participating in peer review and supervising students where required.

ABOUT YOU

You are an experienced diabetes clinician and an accredited Nurse Practitioner who is passionate about innovation technology-enabled care and improving outcomes for people living with diabetes.

You bring a strong clinical foundation deep knowledge of diabetes management and a commitment to delivering inclusive person-centred care. You enjoy building relationships collaborating across teams and contributing to a high-performing evidence-driven service.

  • Advanced clinical skills in diabetes assessment management prescribing and structured education.
  • Strong capability in interpreting digital glucose data and supporting safe effective use of diabetes technology.
  • Experience delivering diabetes education to consumers health professionals and/or community groups.
  • Excellent communication documentation and report-writing skills with the ability to build strong clinical and community relationships.
  • A commitment to equity-informed care and experience working with diverse communities including CALD and Aboriginal and Torres Strait Islander peoples.

Essential: Nurse Practitioner registration with AHPRA Credentialled Diabetes Nurse Educator Police Check WWCC and Victorian Drivers Licence CPR/First Aid.

Desirable: OzDAFNE / Cert IV TAE Minimum 5 years in diabetes education or diabetes clinical care.
