Position Summary
Navitas Healthcare LLC is seeking Social Worker LCSWfor an exciting job in Hyannis MA.
Responsibilities
Conduct behavioral health assessments and develop Initial Treatment Plans.
Lead treatment planning discharge coordination and aftercare arrangements.
Provide group therapy individual counseling and family sessions.
Assess medical psychosocial and home-care needs; link patients to community resources.
Coordinate crisis prevention/intervention and serve as liaison with community agencies.
Secure insurance authorizations and maintain regulatory compliance (CMS DPH DMH).
Provide coverage for inpatient and partial hospital behavioral health programs.
Deliver high-quality service and participate in outreach as needed.
Qualifications
MA Licensed Clinical Social Worker (LCSW).
CPI training and BLS certification required.
Strong communication and clinical judgment.
