Now Hiring: Director of Care Management
Are you a dynamic nursing leader passionate about improving care coordination and patient outcomes Were seeking a Director of Care Management to lead a high-performing team focused on quality collaboration and compassionate care over 2 thriving acute care hospitals in the Pacific Northwest region.
In this pivotal role youll oversee care management and utilization review functions that ensure every patient receives safe seamless and cost-effective care from admission through discharge. Youll guide a talented interdisciplinary team and partner closely with clinical and operational leaders to elevate the patient experience and optimize performance across the continuum of care.
What Youll Do:
- Lead care coordination discharge planning and utilization review processes.
- Drive systemwide initiatives that support clinical excellence and efficient resource use.
- Coach and mentor a skilled team of nurses and care management professionals.
- Collaborate with physicians community partners and payors to ensure smooth transitions of care.
- Use data and analytics to track trends improve outcomes and support organizational goals.
What Youll Bring:
- Bachelors degree in Nursing (Masters preferred).
- RN with at least 2 years of acute care experience.
- 3 years in care coordination or utilization review leadership at the Director level.
- A visionary relationship-centered approach to leadership and change management.
- Commitment to advancing quality equity and patient-centered care.
