We are seeking a dedicated and experienced RN / Patient Care Manager in Home Health to join our team. This role is central to ensuring our patients receive high-quality compassionate care in the comfort of their homes. The ideal candidate will possess strong leadership skills clinical expertise and a commitment to improving patient outcomes.
Oversee the day-to-day clinical operations of the home health team ensuring compliance with healthcare regulations and standards.
Coordinate patient care plans working closely with physicians therapists and other healthcare providers to deliver comprehensive care.
Supervise and support nursing staff providing guidance and assistance as needed to maintain high standards of care.
Conduct regular evaluations of patient care services implementing improvements to enhance service delivery.
Facilitate communication between patients families and healthcare providers to ensure clear understanding of care plans and expectations.
Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
Qualifications: Registered Nurse (RN) with a valid nursing license in the state of operation.
Minimum of 3 years of nursing experience preferably in a home health setting.
Prior experience in a managerial or supervisory role is highly desirable.
Strong organizational and communication skills.
Ability to work independently and manage multiple priorities effectively.
Commitment to maintaining the highest standards of patient care and confidentiality.
