This position focuses on providing compassionate hospice care and patient support across Glen Carbon and surrounding areas. The role involves managing and delivering individualized care plans ensuring timely documentation and maintaining a supportive presence for patients and families during end-of-life care.

Key responsibilities

Manage and deliver patient care plans in hospice settings
Support patients and families within the assigned region
Complete charting and documentation within established deadlines
Manage time and schedule independently

Schedule

Saturday and Sunday 8:00 a.m. to 8:00 p.m.
One additional day (Friday or Monday) from 8:00 a.m. to 5:00 p.m.
Candidates available for only Saturday and Sunday may also be considered

Travel

Local travel within the assigned area

Qualifications
Required

Associate degree or higher in Nursing
At least 2 years of hospice experience
Active Registered Nurse (RN) license in Illinois
Valid drivers license reliable transportation and current auto insurance

Preferred

Provide RN license number for credential verification
