YES Service (YESS) is a registered NDIS provider that specialises in Support Coordination. Our mission is simple: to empower people living with disability to live the lives they choose. We work closely with participants who have a wide range of disabilities and psychosocial needs helping them build independence wellbeing and meaningful community connections.
Due to continued growth across Queensland were on the lookout for two Support Coordinators to join our expanding teamservicing the Southern Brisbane/Gold Coast region.

As a Support Coordinator youll be supporting NDIS participants to get the most out of their plansconnecting them to the right services building their capacity and helping them navigate challenges with confidence. Your days will include:
Working directly with participants to set and achieve their goalsLinking clients with disability and mainstream servicesResponding calmly and effectively during times of crisisMaintaining accurate and timely records reports and progress notesOnboarding new clients and developing tailored coordination plansBuilding strong local networks with service providers and community supportsAbout You
Were looking for people who are driven compassionate and love helping others succeed. Youll need:
A strong understanding of the NDISHigh emotional intelligence and excellent interpersonal skillsStrong admin documentation and time management skillsExperience managing a caseload in a billable-hours environmentA qualification in disability community services or relevant lived/professional experienceThe ability to work independently from home and as part of a supportive remote teamA current drivers license and willingness to travel within your regionYoull Also Need:
