Key Responsibilities
Deliver skilled nursing care and comprehensive patient assessments in home settings
Develop and implement individualized care plans in collaboration with the healthcare team
Educate patients and caregivers on treatment plans medications and home safety
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely clinical documentation in accordance with policies and regulations
Communicate effectively with physicians and interdisciplinary team members to ensure coordinated care

Schedule and Shift Details
Monday through Friday daytime schedule
Minimal weekend shifts per year
Flexible scheduling with a supportive work-life balance
This is a Pay Per Visit position

Travel
Local travel required within the Carlisle area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior home health experience
Strong assessment time management and communication skills
