Community Support Specialist

The Community Support Specialist is responsible for providing a wide variety of community based support services to individuals with a Serious Mental Illness (SMI) designed to reduce and/or prevent emergency department visits mental health unit admissions or readmissions and placement in a more restrictive environment.

Responsibilities
- Provides daily living skills training including but not limited to development of medication management strategies shopping learning the transportation system symptom management skills locating housing coping strategies transportation to selected appointments etc.
- Provides a wide variety of care coordination or case management activities including but not limited to information and referral education setting up appointments setting up transportation working with others involved in the clients life responding to crises etc.
- Documentation of all services provided in BHGMHCs Electronic Health Record according to BHGMHC standards insurance and/or managed care company requirements state standards etc.
Qualifications
Education:
- Bachelors degree in a human-service related field required
Experience:
- 1-3 years experience in the mental health field
