As a Hospice C.N.A. youll be part of a mission-driven team providing dignity comfort and support to patients and families during lifes most meaningful moments. Your presence your care and your heart will make a lasting impact.
Schedule: MondayFriday 8:00am5:00pm

Provide hands-on compassionate care following each patients care plan
Offer comfort dignity and emotional support during end-of-life care
Assist patients and families in learning self-care skills
Report updates to the RN Case Manager and care team
Document care provided changes in condition and patient needs
Help prevent unnecessary hospitalizations and support continuity of care
High School Diploma or GED
1 year of experience as a Nursing Assistant or Hospice Aide
Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR 484.36)
Hospice or home health experience preferred
Reliable transportation valid drivers license and auto insurance (50% travel)
Ability to lift 50100 lbs and perform hands-on caregiving duties
