Department: Toronto Western Family Health Team
Work Model: On-Site
Reports to: Clinic Manager
Hours: 37.5 hours per week
Shifts: Days Occasional Evenings and Weekends
Status: Temporary Full Time (Maternity Leave Contract)
Closing Date: August 22 2025

Position Summary
Toronto Western Family Health Team (TW FHT)s program links with the surrounding community to provide primary care to families and community members with special attention to the needs of the aging population children and women and patient populations who include: a) underserved at-risk and high needs patients c) adolescents and emerging adults and d) patients with mental health and addictions.

With the teamwork of physicians nurse practitioners primary care nurses social workers pharmacists physiotherapists occupational therapists respiratory therapists and chiropodists.

Duties

  • Work collaboratively with members of the team in providing support to patient care and clinic flow
  • Escort patients from waiting area to assigned examination room
  • Takes vital signs as per clinic guidelines
  • Assist patients with way finding to check in and out
  • Relay patient concerns and changes to health care providers
  • Assist with clinic day-to-day functions including:
    • Clinic set up and performs end of day functions
    • Cleans procedure trays as per hospital procedure and prepares transportation to MDR
    • Provides specimen handling storage and communication with external lab
    • Maintains general tidiness of cleaning/soiled utility rooms
  • Ensure clinic supplies are ordered functioning and available

Qualifications :

  • Completion of 1 year and over in Personal Support Worker Hospital Assistant and / or Healthcare Aide certificate(s)
  • Completion of a Basic Cardiac Life Support (B.C.L.S) program
  • Current registration with the Ontario PSW registry
  • 1 year practical and related experience and/or 6 months on-the-job training
  • Previous work experience in a healthcare setting with primary care setting strongly preferred
  • Excellent communication and interpersonal skills
  • Knowledge of the Workplace Hazardous Materials Information System (WHMIS)
  • Ability to work independently as well as in a team
  • Ability to lift over 20lbs
  • Ability to work efficiently and effectively under pressure
  • Demonstrated to use good judgement
  • Experience working with a diverse community population
