This Opportunity:

The primary responsibility of the Support Coordinator is to provide support to program supervisors administrative staff Direct Support Professionals and the individuals that are supported by the agency. This job provides leadership for Direct Support Staff in mentoring and growing their skills and provides support in planning coordinating scheduling and directing the day-to-day functions of assigned programs that will enhance the quality of life through services that enable individuals to achieve their personal dreams and goals within their community.

What Youll Do:

Provide leadership direction mentoring and supervision (in the absence of immediate supervisor) to assigned personnel including floaters and substitute staff.
Maintain staffing schedules that meet the ratios required for service provision. Position will be a fill-in staff as required.
Support staff to ensure person-centered and positive approach strategies are being used to support individuals receiving services.
Identify methods whereby services can be provided utilizing existing community resources wherever possible.
Work in a cooperative manner with involved professionals to ensure that program plans are implemented and documented properly and updated in a timely fashion.
Serve as a resource for person served on community agencies services and supports that can meet identified needs or goals.
Attend and actively participate in meetings and trainings as required. Maintain current certification in all required training as outlined by agency policy and/or state and federal guidelines.
Continually assess each person receiving services for their hopes dreams and desires and provide supports that facilitate achievement.
Ensure that all required paperwork/documentation and reports are completed by DSP. Report any concerns to professionals or supervisor.
Assist in maintaining the records of the individuals supported.
Supervise and monitor housekeeping staff where appropriate.
Monitor performance and implementation of assigned responsibilities by being present when day-to-day activities are taking place.
Ensure that the rights of each person receiving services are protected and promoted at all times.
Assist people in developing advocacy skills participating in advocacy efforts and utilizing personal advocates.
Maintain a safe working environment for employees and people receiving services by practicing safety procedures in the community and on the job.
Attend and actively participate in meetings and training as required. Maintain certification in all agency state and federal training requirements.
Demonstrate knowledge of and comply with all agency policies and procedures as well as state and federal statutes and regulations related to specific program areas
Demonstrate knowledge of emergency procedures and assist in crisis situations.
Complete all other relevant responsibilities as assigned by the supervisor.
Travel and driving may be required.

*The definition of an Innovations direct care worker includes all workers required for compliance with or delivery of the relevant Innovations waiver service definitions and the delivery of a unit of Innovations services to individuals in the definition of direct care worker to be applied and shall include only caregivers who are contracted for the Page 230 Session Law 2023-134 House Bill 259 provision of services in a legally appropriate manner.

Schedule:

Monday-Friday (8:00am-5:00pm)
