About the Role:

  • Permanent full-time position providing crisis assessments interventions counselling and psychoeducation while coordinating access to psychosocial supports and community services tailored to individual needs in acute care services.

  • Proactively monitor and respond to changes in consumer risk status ensuring timely clinically appropriate escalation and intervention.

  • Foster strong collaboration with internal teams and external partners including health professionals crisis accommodation services and the Homeless Health Assertive Outreach team to ensure seamless coordinated care.

  • Champion consumer advocacy by addressing personal needs concerns and preferences in full compliance with the NSW Mental Health Act (2007; amended 2015).

  • Support a culture of learning and improvement by contributing to staff development delivering training within the multidisciplinary team and participating in ongoing service enhancement initiatives.

About You:

  • Degree in Social Work eligible for membership of the Australian Association of Social Workers (AASW) and a full Australian drivers licence.

  • Experience in community mental health with confidence in acute and triage settings risk assessment and crisis intervention.

  • Committed to holistic person-centred practice that includes families and carers as part of the care team.

  • A natural collaborator who builds strong purposeful partnerships across services to deliver seamless compassionate wraparound support for complex caseloads

  • Youre highly organised with excellent communication skills and confidence using electronic medical records to manage complex caseloads.
