This leadership role will focus on designing and implementing a robust survivorship program to include clinical oversight with seamless transition across the cancer care partnership with the Program Director the leader will support the vision and meeting the goals of the Survivorship & Cancer Outcomes Research (SCOR).

Job Description

Program Operations & Compliance

  • Oversee all operational aspects of the Cancer Value Based Care & Enhanced Oncology Model (EOM) participation ensuring adherence to CMS guidelines reporting requirements and program timelines.
  • Maintain thorough understanding of Cancer Value Based Care & Enhanced Oncology Model (EOM) requirements updates and policy changes; proactively communicate implications to internal stakeholders.

Care Transformation Support

  • Partner with clinical leadership to develop implement and monitor care delivery redesigns aligned with EOM goals (e.g. care coordination patient navigation financial counseling palliative care integration and psychosocial support services).
  • Collaborate with providers nurses social workers navigators and support staff to ensure timely delivery of EOM-required enhanced services.
  • Facilitate best practice sharing across participating oncology practices.

Data Management & Analytics

  • Coordinate with analytics teams to develop dashboards and reports that monitor performance on EOM quality measures cost benchmarks and care delivery metrics.
  • Analyze trends identify care gaps and work with teams to develop targeted interventions for performance improvement.

Financial Management

  • Monitor financial performance of the Cancer Value Based Care & EOM program including Monthly Enhanced Oncology Services (MEOS) payments performance-based payment reconciliation and budgeting for enhanced service delivery.

Stakeholder Engagement & Education

  • Facilitate regular interdisciplinary meetings to review program performance address barriers and celebrate successes.
  • Provide monthly/quarterly updates at QAPI/Hem Onc Division Meeting/etc.
  • Provide ongoing education and training to clinical teams administrative staff and leadership regarding EOM program components requirements and expectations.

Additional Job Description

Qualifications:

  • Bachelors degree
  • Experience in working on interdisciplinary teams required
  • Willingness to be crossed trained to other areas
  • Good written and verbal communication; as well as organizational skills required
  • Ability to communicate clearly and concisely with all levels of nursing administration community members and physicians
