Job Summary:
Under the direction of the Director of Inpatient facilitate patients through the entire continuum of care through ongoing clinical evaluation and management in collaboration with the entire patient care team while complying with Hospital policies. Collaborate with the clinical team and post-acute providers to advocate for supervise and support patients through the entire episode of care to promote better outcomes and prevent unnecessary readmissions and complications during the 90-day recovery period. Acts as a liaison between the patients family physicians nursing and ancillary services. Collect program outcome data and evaluate the effectiveness of the Spine Program. Coordinate patient and community education/outreach as well as hospitality and patient satisfaction. Directs and provides professional nursing care for an assigned group of patients across the age continuum while complying with Hospital policies. Utilizes the nursing process of assessment planning implementation and evaluation to direct or provide nursing care. Coordinates and monitors the provision of hospital services by exercising leadership creativity initiative and cooperative problem solving in the areas of Spinal procedures and policy management. Serves as a resource person that develops implements and monitors program initiatives to assure compliance with local state federal and Joint Commission standards.
Provides care and services appropriate to the age and needs of patients served. Provides necessary information and education to the patient for: preoperative intra-operative recovery discharge and post discharge processes. Effectively manages the patients transition from the hospital to other settings. This includes working in conjunction with outpatient Physical Therapy Home Health agencies skilled nursing facilities and Acute Rehab Facilities to ensure patients follow a coordinated plan of care.
Acts in a leadership role with the interdisciplinary healthcare team to coordinate patient and community education and outreach as well as hospitality and patient satisfaction. Promotes and facilitates discharge planning. Identifies and intervenes when necessary to ensure efficient and cost-effective delivery of patient care services. Establishes and promotes collaborative relationships with physicians patients families and additional members of the healthcare team. Educates members of the healthcare team on complete joint replacement spine procedures and readmission rates. Acts as a resource for routine and complex problem solving.
Projects a professional demeanor and appearance while maintaining the confidentiality of patients co-workers and the facility and adhering to the HIPAA policy. Reports in good faith any known or suspected activity that appears to violate laws rules regulations or the Surgery Partners Code of Conduct. Fulfills annual continuing education requirements.

Work is of light demand. Visual and auditory acuity and manual dexterity essential to performing designated duties required. Optimal auditory acuity required. Manual dexterity involving the handling of computers and other equipment is essential to performing assigned duties. Physical conditions are clean neat and well-lit. May be subjected to unpleasant sights and odors stressful situations and hazardous or infectious agents where judgment as to precautions needed to be taken is essential. Climate control and ambient temperature variances may be experienced and required by patient care and needs of the surgical team. Exposure to limited amounts of radiation hazardous chemicals or substances and infectious disease processes are a possibility. Hours of duty may be irregular or unexpectedly extended due to the requirements of the daily schedule.
