At Boys & Girls Haven we provide a Home and a Future for some of Kentuckys most vulnerable and at risk youth. As a nonprofit foster care agency we exist to Shelter Heal and Teach children and young adults to become resilient members of our community through an integrated spectrum of care.

Role Summary: Provides planning implementing monitoring and evaluating actions required to meet the youths overall treatment needs. Care Coordinator is also responsible for managing referrals and facilitating the client intake process for residential BGH addition s/he has the task of managing the Extended Reach EHR system and the initial client file process and is responsible for the care of the file throughout the clients stay within the residential program. S/he is also responsible for administering intake elements of all clients tracking programs for Department for Community Based Services (DCBS) Childrens Review Program (CRP) Department for Juvenile Justice (DJJ). Assures that reports are sent to the appropriate source through periodic reminders sent to staff. Must potentially be available 24 hours a day depending on client/referral source needs.Essential Functions: Responsible for coordinating and connecting care for an assigned case load of youth within the Residential Program. - Assessing the youths and familys needs ensuring appropriate connections to treatment team members and outside resources are initiated and followed up on. Ensuring outside treatment team members are contacted within 3 days of youths admission to ensure strong connections are made with all treatment team members.
- Serves as the initial point of contact for a variety of referral sources including CRP placement coordinators DCBS workers parents and community partners
- Communicate and facilitate with appropriate treatment team members youths and familys needs treatment concerns and safety issues as outlined in BGH Policy and Procedures. (i.e AWOLs bed holds clothing needs medical hospitalization CSA/TSS services approval for homes passes etc.)
- Open and maintain case files for youth that include all documents required. Make sure youth and family understand program requirements goals and treatment plans as well as grievance forms clients rights release of information and other relevant forms.
- Ensure clients complete necessary intake and ongoing paperwork. Note the clients progress performance and achievements so this information can be used when compiling case notes case review/audit documentation progress reports and other critical documentation.
- Schedules and completes interviews pre-placement visits and admissions.
- Responds to referral sources within 24 hours of receiving referrals by phone fax email or through CRP.
- Uses discretion and independent judgment in determining appropriateness of referrals to BGH. Collaborate with Clinical Director or other appropriate staff and prepares referrals for review and final admission decision.
- Sets up intake for accepted referrals; completes initial paperwork with client and referral source(s).
- Responsible for monthly and weekly connections to DCBS Aetna and other possible outside supports (i.e. 1294a 1294b CRP Level of Care Application treatment team meetings etc.).
- Collaborate and implement discharge plans with treatment team from the beginning of admission. Exploring and connecting additional outside supports for youth and family to be successful after discharge.
- Responsible for running URC biweekly meetings and collaboration efforts within BGH programs
- Assist with transition from one program within BGH to another via appropriate documentation and communication.
- Completing documentation at discharge to ensure representation of all needs are noted for a successful transition for youth and family.
- Manages census and the whereabouts of our clients via Excel spreadsheets as well as various databases and systems.
- Sends admission/discharge notifications to program managers or their designees.
- Organizes computes and prepares appropriate weekly monthly quarterly and annual reports
- Meets regularly with the immediate supervisor as a means of enhancing professional growth reviewing and processing the provision of care coordination services and dealing with appropriate administrative issues.
Unplanned Activities: At BGH we value teamwork. To meet departmental and organizational goals the employee is expected to assist with activities that are not formal parts of the roles essential functions and may be considered other duties as assigned.Qualifications:- Education - A Bachelors degree in social services or other related/relevant field.
- Experience/Credentials Targeted Case Management (TCM) certification or will acquire training certification within six months of hire. Minimum of two years experience working with older adolescents and young adults in a residential treatment mental health/psychiatric hospital or pre-independent/independent living setting. Excellent verbal and written communication and organizational skills including proficiency with appropriate computer software/hardware (i.e. Windows Microsoft Excel Outlook Power Point Word and other relevant software/hardware).
- Other - Interact professionally with BGH employees clients visitors government agencies and the general public. Maintain confidentiality of all client information. Must be at least 21 years of age and legally able to work in U.S. Valid drivers license and insurability by BGHs insurance carrier required. Successful completion of all background screenings and pre-employment drug screen. Successful negative post-contingent offer and annual TB screenings. Safe Crisis Management training and maintenance of certification through BGH.
Physical Demands: While performing the duties of this job the employee is required to: stand walk and sit; use hands to manually manipulate handle or feel objects tools or controls; reach with hands and arms; climb stairs; balance; stoop kneel crouch or crawl; talk and hear; taste or smell. The employee must occasionally lift and/or move up to 40 pounds. Specific vision abilities required by the job include close vision distance vision color vision peripheral vision depth perception and the ability to adjust focus. Ability to read write and converse in English.
Work Environment: While performing the duties of this job the employee is exposed to clients apartments communal areas and a standard office environment. Clients living spaces may range from impeccably clean to less than appealing regarding order and cleanliness. The noise level in the work environment is usually moderate. The employee will come into contact with BGH clients staff members Board members volunteers and other members of the community.
