PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Street Outreach team as the Outreach Case Manager at the Downtown Center BID Office location.

ABOUT PATH
Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.

ABOUT OUR TEAM
PATHs Street Outreach programs utilize our street outreach integrated approach to engage with individuals who are currently experiencing homelessness and living on the streets. The goal of these programs is to quickly assess and link individuals with the supportive services and resources necessary to improve their well-being and quality of life transition off of the streets and obtain and sustain housing long-term.

ABOUT THE JOB
As part of the Street Outreach team the Outreach Worker plays a key role at PATH as one of the first points of contact for homeless clients in the community. They utilize PATHs street outreach integrated approach to provide field-based direct client outreach and engagement and case management services quickly assessing needs and linking clients who are currently homeless and living on the streets with supportive services

Position Responsibilities include:

  • Conduct daily street outreach to develop relationships and build rapport with clients in order to assist them with finding a home.
  • Conduct VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) on all new clients in order to connect them to proper intervention and services.
  • Document results complete match initiation form and submit all paperwork to CES Coordinator.
  • Meet with established clients on a weekly basis to review evaluate and support goal completion.
  • Provide street-based mobile case management services designed to connect homeless clients to long-term supportive services:
    • Assist clients with the establishment of personal benefits including (GR SSI TANF Veterans Pension etc.).
    • Help clients obtain various forms of identification including birth certificates and social security cards.
    • Provide employment assistance housing referrals and health related assistance as identified on the ISP.
    • Assist with applications for supportive and subsidized housing and prospect potential locations for affordable rental housing.
    • Provide ongoing information referrals linkages and advocacy for all other identified needs

WHAT YOU BRING
Were looking for candidates that:

  • Ability to work independently and within a team environment and exercise mature judgment.
  • Excellent interpersonal skills ability to work in a variety of settings with culturally diverse persons and communities with the ability to be culturally sensitive and appropriate.
  • Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations.
  • Excellent leadership skills with a hands-on lead-by-example work style.
  • Highly motivated self- starter with the ability to coordinate multiple projects/ tasks simultaneously in a high-pressure environment.
  • Demonstrated knowledge and experience with evidenced based interventions:
    • Engagement and rapport building with hard-to-reach clients.
    • Harm Reduction and Motivational Interviewing strategies.
    • Housing First.
    • Non-violent crisis intervention techniques.
    • Case management interventions that have been proven effective in vulnerable populations.

PREFERRED QUALIFICATIONS

  • Two (2) years of experience in healthcare case management homeless services or working with vulnerable populations or a related field.

MINIMUM QUALIFICATIONS

  • All level of education and experience welcome.

MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:

  • Have reliable transportation
  • A valid drivers license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATHs insurance coverage
